No human is perfect. We all make mistakes. However, sometimes humans make very costly & unhealthy errors, leading to disastrous consequences. Which of these do you think was the WORST mistake made by a human?

The first three options are more mistakes rather than just “decisions” and the Chernobyl disaster was due in large part to poor construction of the plant and the allowing of operators with little knowledge of nuclear physics, in addition to the poor decisions made by those operators.

If using NASA decisions, the decision to launch Challenger would have to be the example. The mixup between standard/metric was a mistake, Challenger was a bad decision, and when you read more about the factors that day, an evil decision. Despite warnings from engineers that the tank would blow up, they went ahead with the launch anyway for PR reasons.
